PUBLIC RECORDS REQUESTS

It is the policy of Mid-Valley Hospital to provide for access to public records as required in RCW 42.56 and any other applicable provisions of federal or state law.

Per policy, requests for inspection of records shall be made available Monday through Friday, 9:00am to 5:00pm, except on holidays. There is no charge for the inspection of records.

If the request is for copies of records, MVH shall charge $0.15 per printed page and any actual mailing costs. We do not charge for electronic format of records sent by e-mail. If electronic copies of records are requested to be loaded to a disk, MVH will charge the cost of the computer disk. All charges for copies of records and/or disk, and appropriate mailing costs, shall be paid prior to the release of the records.

Requests for access to public records may be delivered by mail, telephone, or personal delivery. Mailed requests shall be addressed to:

Telephone requests shall be directed to the Public Records Officer at 509-826-1760, extension 2348. Personal delivery shall be addressed to “Public Records” and signed in at the main registration desk at Mid-Valley Hospital, 810 Jasmine St., Omak, WA.
